This website is operated by Kirstin's Haven, Inc., and this policy explains what information the site collects and how it is used. It covers Kirstin's Haven, Inc. and its sister organization, Kirstin Care, LLC, which provides our outpatient behavioral health services.
Information we collect
We collect only what you choose to send us. When you use the contact form, we receive your name, email address, phone number (if provided), the topic you choose and your message. When you apply for a job, we receive the answers on the application, including your contact details, work history, education, references and any resume you attach. We use this information only to respond to you or to consider your application.
Please do not send detailed medical or health information through our contact form or by email. Patient forms are completed through a separate secure service, described below.
How your form submission is handled
Contact form and job application submissions on this site are processed by a centralized form delivery service at forms.caltechweb.com, operated by CalTech Web. The service delivers your submission by email to our team, attaching any resume you upload, and does not use your information for any other purpose. Each submission also records the page you sent it from, the first page you visited on this site and the website that referred you, so we can understand how people find us.
Spam and bot protection
This site uses Cloudflare Turnstile on the contact form to verify that a submission is coming from a real visitor rather than an automated script. Turnstile may process limited technical information about your browser and device as part of that verification. See Cloudflare's own privacy policy for details on how Turnstile handles that data.
Analytics
This site uses Google Analytics 4 (GA4) to understand how visitors use the site, such as which pages are viewed and how visitors arrived here. GA4 uses cookies and similar technologies to collect this information. You can opt out of Google Analytics tracking using the Google Analytics Opt-out Browser Add-on.
Other third-party services
- Google Maps. The contact page embeds a Google map of our office. Google may set cookies and collect usage information when the map loads, under Google's own privacy policy.
- PayPal. Online donations are made on PayPal's website. We never see or store your card details; PayPal's privacy policy applies to the information you give it.
- HushForms. Our new patient forms are hosted by HushForms, a secure form service. Information you enter on those forms goes to Kirstin Care, LLC and is protected as health information.
This site has no chat, booking or review widgets.
Cookies
This site uses cookies set by Google Analytics and Cloudflare Turnstile and the Google Maps embed on our contact page as described above. You can control cookies through your browser settings.
Notice of Privacy Practices
Kirstin Care, LLC is a health care provider covered by the Health Insurance Portability and Accountability Act (HIPAA). Its Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to it. You receive a copy of the Notice at intake, and you may ask for a copy at any time by calling (240) 392-2876 or visiting our office.
Under HIPAA, you have the right to:
- See and get a copy of your health records
- Ask us to correct your health records
- Request confidential communications and ask us to limit what we use or share
- Get a list of those with whom we have shared your information
- Get a copy of the Notice of Privacy Practices
- File a complaint if you believe your privacy rights have been violated
You can file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Children
This website is not directed to children under 13, and we do not knowingly collect their information online.
